PRIVACY POLICY AND PERSONAL INFORMATION GOVERNANCE PRACTICES

The Dimensions Santé Clinic complies with the requirements of Law 25.

At Clinique Dimensions Santé, we attach paramount importance to the confidentiality and security of our patients' information. This privacy policy is intended to inform you about how we collect, use, and protect your personal information.

What is personal information?

The Privacy Act (Privacy Act) defines personal information as follows: “any information that relates to a natural person and allows, directly or indirectly, the individual to be identified.”

Personal information can be a person's name, first name, date of birth, telephone number, address, social insurance number, etc.

Data collection

At Clinique Dimensions Santé, we collect personal information when you make an appointment (online or by phone), fill out medical forms, or interact with our staff as part of your treatments. They may also allow us to promote our services, for example with the use of Google Ads (a way of promoting our services in connection with your searches on Google.com).

Only the information necessary for these purposes is collected, with your consent.

Data protection

We take appropriate security measures to protect your personal information from unauthorized access, disclosure, alteration, or destruction.

Our IT systems are secure and we follow best practices in terms of protecting computer and physical data (e.g. via technological measures such as firewalls with IPS system, antivirus + EDR, 24/7 monitoring of the Microsoft 365 environment, encryption, encryption, SSL certificates, locked premises and networks, etc.). In fact, our appointment scheduling and record keeping software is SOC2 certified by Raymond Chabot Grant Thornton, guaranteeing that all necessary policies and procedures are in place to protect your information. No copies are kept outside of these locked applications and/or environments.

All clinic staff are trained on privacy policies and procedures, as well as data protection best practices. We ensure that staff members who have access to certain information requiring their protection sign strict confidentiality agreements.

However, sharing information via the Internet is not without risks. As a user, you should protect your personal information by making sure you create secure passwords and by keeping your username/password confidential at all times.

We are committed to informing you of any potential or real leaks that may occur.

Information sharing

We only share your personal information with third parties in the following cases: when required by law, or when you have given us your explicit consent to do so and when this is necessary for your treatment or for other purposes (for example, with other health professionals involved in your care or with third party payers such as CNESST, SAAQ, private insurance, etc.). The only people who have access to your personal information are those who are authorized to do so and for whom it is necessary in the course of their duties.

Your information may be shared with service providers who are governed by confidentiality agreements and who are committed to complying with privacy law (e.g., information technology/website, maintenance, application development company, others). Their access remains limited to the strict minimum necessary for their function.

Data Retention

We keep your personal information for as long as necessary to provide the services requested and to comply with our legal and regulatory obligations. We securely destroy any personal information that is no longer required.

Your rights

You have the right to access, correct, update, or remove your personal information. You can also object to the processing of your personal data in certain circumstances.
